What you can do with custom forms
A good form is the cheapest employee you'll ever hire — it works at 2am and asks everyone the same questions. Flowpera gives you fifteen ready-made templates, a public page for each, and a direct line into your contact lists so no submission is ever stranded.
Start in seconds
Pick from fifteen templates grouped by category — contact, lead capture, onboarding, RSVP, feedback and more — each pre-filled with sensible questions, or build your own from scratch.
Every field type you need
Short text, long text, dropdowns, single-choice and checkboxes, each markable as required, in whatever order tells your story.
Share and embed
Every form gets a clean public page, a direct link, and an embed snippet you can drop into any website — no developer required.
Straight into your CRM
Map each form field onto a contact list and every submission becomes a contact, deduped by email, ready for follow-up. Missing a column? Create one from the mapping screen.
See every response
An entries view collects submissions across all your forms, while a settings overview shows each form's status, public link and where it syncs.
On and off as you go
Turn forms active or paused as campaigns come and go — a paused form stops accepting submissions without losing any of its history.
Charge for a submission
Attach a product to a form to take payment on submit — required up front, or invoiced to pay later — so an order form, a deposit or a paid application just works. Runs on your own Stripe account.
See how your form performs
Each form tracks its visits, so its Share tab shows total and unique views alongside a conversion rate from views to submissions.
Up and running in a few steps
- 1
Pick a template
Choose from fifteen pre-made forms grouped by category, or start from a blank canvas and add exactly the questions you want.
- 2
Shape your questions
Add text, dropdown, choice and checkbox fields, mark the important ones required, and order them the way you'd ask in person.
- 3
Map to a contact list
Connect the form to a list and map each field to a column — creating a new column on the spot if nothing fits — so every answer has a home.
- 4
Publish and collect
Share the link or embed the form. Each submission is stored and, when mapped, becomes a deduped contact in your CRM automatically.
See every response in one place
An entries view collects all submissions across your forms, while a settings overview shows each form's status, public link and where it syncs. Turn forms on and off as campaigns come and go, and map every field so nothing a respondent tells you ever falls through the cracks.
- All submissions in a single entries view
- Per-form status and sync overview
- Toggle forms active or paused anytime
Built for the way you work

Event organizers
Collect RSVPs with a public form, then work the guest list straight from your CRM without exporting a single spreadsheet.
Support & operations
Run an intake or feedback form, map answers onto the fields you track, and pick up each response from one shared entries view.
Questions, answered
Can I embed a form on my own website?
Yes. Every form has a public page, a direct link and a copy-paste embed snippet that drops it into any site, no code beyond a paste.
What if a form field has no matching contact column?
Create one right from the mapping screen. You can add a new column to the target list on the spot, so every field you collect has somewhere to be saved.
Do submissions still save if I don't connect a list?
Always. Every submission is stored and visible in the entries view whether or not the form is synced to a contact list.
Can I pause a form without deleting it?
Yes. Toggle a form to paused and it stops accepting submissions while keeping every past entry and its settings intact.
Can a form take payment?
Yes, once you connect Stripe. Attach a product to the form and choose whether payment is required before the submission is accepted, or an invoice is emailed to pay later. See Products & Payments for the details.
