Custom Forms

Custom forms are general-purpose forms with no time-slot picker — contact, lead, RSVP, survey and more. Each gets a public page and maps its fields onto a contact list. This guide covers building, publishing and syncing them.

Templates and categories

Start from one of 15 pre-made templates or a blank form. Templates are grouped by category in the picker, which pre-fills the questions for you.

Note

Forms have no dedicated date input, so any template that needs a date uses a text field whose label states the expected format.

Building your questions

Add fields in the order you'd ask them in person. Each can be marked required.

Short text

A single line for names, companies or references.

Long text

A paragraph for detailed answers.

Dropdown

Choose one option from a list you define.

Single choice

Radio-style selection of exactly one option.

Checkboxes

Select any number of options.

Publishing and embedding

Public page

Every form has a clean public page at its own address.

Direct link

Copy and share the link anywhere.

Embed code

Paste the snippet into any website to embed the form inline.

Active / paused

Pause a form to stop new submissions without losing any history.

Link tracking

The Share tab shows total and unique views plus a conversion rate from views to submissions.

Mapping fields to a contact list

Custom forms use field-centric mapping: every field maps onto a column in a target contact list, and submissions dedupe by email.

  1. 1Choose the contact list this form should feed.
  2. 2Map each field to a column — the editor auto-matches by label.
  3. 3Create a new column inline when nothing fits, or leave a field on "Don't sync".
Note

Submissions are always stored whether or not the form is connected to a list, so you never lose a response by leaving it unmapped.

Viewing submissions

Every submission records the submitter's country, device type and time.

A per-form status and sync summary showing where each form points and whether it's active.

Charging for a submission

A custom form can take payment by attaching a product on its Payment tab — turning it into an order form, a paid application or a deposit form. Payment runs through your own connected Stripe account.

No payment

The default — a free form.

Invoice to pay later

The submission succeeds immediately and an invoice for the attached products is emailed to pay later.

Require payment

The visitor pays through Stripe before the submission is accepted.
